Notes for completing application form.
Please print in block capitals
(1) Name : Please print your name in full
(2) Title: Mr, Mrs Ms, Dr, Rev etc.
(3) DOB: Date of birth (in the form DD/MM/YY)
(4) Address: Full postal address for correspondence. Print your E mail address, to make it clear and provide a contact telephone number where you can be reached if there is a problem with your placement.
(5) University course and year:
University attended Title of the course you are studying Year of the course you are currently completing
This section may not be relevant to everyone.
(6) Archaeological experience:
Please list any relevant archaeological experience. This can include voluntary work, course work or time in paid archaeological employment. It would be helpful if you can list the skill you have acquired such as drawing, context recording on records sheets etc.
(7) Relevant qualifications:
These may include archaeological qualifications, a valid driving licence (UK) or First Aid qualifications.
(8) Which part of the Project particularly interests you? What aspects of archaeology or historical periods are of particular interest to you.
(9) State your preferences for your date of arrival and date of departure. (NB. All dates are subject to availability). Please inform the Project of the day you intend to arrive, not the day you intend to start work. If you arrive early, without prior notice, there will not be food available.
(10) Medical conditions: These may include food allergies, asthma epilepsy etc. All information will be treated confidentially.
(11) Special dietary needs: A vegetarian food option will be provided normally. If you have any further requirements the project may not be able to cater for you.
(12) Further information: Please use the space provided to add any comments or questions that you have or add any further information you feel is relevant.
